Calling Miss Manners

At what point do you just bite the bullet and admit you don't know the last name of somone you've known for months and months?

There are people I have known for years and still don't know their last name. Sometimes it just never comes up. I think that's reasonable.
It's when you still don't know their first name that it gets embarassing.
